<dc:description><p dir="ltr">Neuroinflammation may arise in the earliest stages of Alzheimer's disease, before the brain tissue loss that current imaging detects. We applied advanced diffusion MRI microstructure models to the parahippocampal cingulum, a memory-related white matter tract, in 299 participants from the New Zealand Dementia Prevention Research Clinics, spanning cognitively unimpaired controls, subjective cognitive decline, mild cognitive impairment and Alzheimer's dementia. Three measures were derived: microstructural complexity, tissue diffusivity and fibre coherence. Complexity peaked early, in participants reporting cognitive concerns but testing normally, then declined in later groups - a pattern found only in those with amyloid pathology. Diffusivity rose steadily across groups, while fibre coherence remained intact until mild cognitive impairment. Only the complexity pattern persisted after accounting for Gray Matter atrophy. These findings suggest diffusion MRI can distinguish an early, possibly inflammatory phase from later degeneration, offering a safe and repeatable marker of the earliest brain changes realted to AD.</p></dc:description>
