<dc:description>The discovery of teixobactin in 2015 was a landmark in
the fight
against antimicrobial resistance, introducing a novel antibiotic scaffold
with a unique mechanism of action. Isolated from the previously unculturable
soil bacterium Eleftheria terrae using
the innovative iChip technology, teixobactin initially showed the
remarkable ability to kill pathogens without apparent development
of resistance. Over the past decade, intensive research has unraveled
a complex mode of action: teixobactin dual-targets the essential cell
wall precursors Lipid II and Lipid III, then triggers a cascade of
self-assembly into amyloid-like fibrils on bacterial membranes, sequestering
these precursors and lysing the cell. Commemorating its 10th anniversary,
this review provides a comprehensive analysis of teixobactin’s
journey. We examine key structure–activity relationships (SAR)
derived from the synthesis of numerous analogs that identified critical
residues and moieties required for activity and led to promising derivative
compounds. At the same time, the road from soil to clinic has been
marked by challenges. The very property that underlies teixobactin’s
potent mechanism, its tendency to aggregate, also poses significant
formulation problems, a “potency-formulation paradox.”
Furthermore, the initial resistance-proof promise has been toned down
by conflicting studies: some confirm an exceptionally high barrier
to resistance, while others raise concerns about rapid adaptation
and cross-resistance. On top of these scientific challenges are the
economic and regulatory barriers that affect antibiotic development
today. In sum, this review synthesizes a decade of research, framing
teixobactin not only as a promising antibiotic candidate but also
as an illuminating case study of the scientific, manufacturing, and
economic challenges in bringing new antimicrobials to patients in
the 21st century.</dc:description>
