<dc:description><p dir="ltr">Introduction: Nodal T-follicular helper cell lymphoma (nTFHL) is a rare subtype of peripheral T-cell lymphoma in which cutaneous manifestations occur in approximately half the patients. Case Presentation: We report herein a case of toxic epidermal necrolysis (TEN) in a 62-year-old, male, Japanese patient with nTFHL, not otherwise specified. The patient experienced the development of rapidly progressive, erythematous plaques with blistering and erosion involving more than 30% of the body surface area. Histopathological examination of the skin revealed total epidermal necrosis without lymphoma cell infiltration while immunohistochemical analysis demonstrated depletion of FOXP3-positive cells. The TEN was successfully treated with systemic corticosteroid therapy, plasma exchange, and intravenous immunoglobulin. Although EBER-positive B cells in lymph nodes and EBV DNAemia were detected, their contribution to the TEN remains unclear. Conclusion: This case expands the spectrum of severe, cutaneous manifestations associated with nTFHL and suggests that a reduction of regulatory T cells, previously reported in lymph nodes, may also occur locally in the skin and contribute to the development of cutaneous manifestations.</p></dc:description>
