<dc:description>
Introduction: Schwannomas are benign peripheral nerve sheath tumors arising from Schwann cells and are most commonly encountered in the head and neck region and extremities. Pelvic schwannomas are rare and may create diagnostic difficulty because their clinical and radiological features are nonspecific and may mimic adnexal masses, particularly in female patients.

Case Presentation: A 19-year-old female patient was evaluated for left hip pain persisting for six months. Pelvic magnetic resonance imaging revealed a well-circumscribed cystic mass measuring approximately 9 x 9.5 cm in the left adnexal region, and an adnexal mass was considered in the initial differential diagnosis. Surgical exploration through median laparotomy demonstrated normal uterus and bilateral ovaries, whereas the mass originated from the left pelvic retroperitoneal space and showed close anatomical relationship with the sacral region. The mass was totally excised. Histopathological examination confirmed schwannoma. Postoperatively, the patient developed motor deficit in the left lower extremity. At sixteen-month follow-up, no local recurrence was detected, and the motor deficit persisted.

Conclusion: Pelvic schwannomas are rare benign tumors that may mimic adnexal masses, particularly in young women. Although preoperative diagnosis is often challenging, careful radiological evaluation and surgical planning may improve diagnostic accuracy. In tumors closely related to neural structures, a careful balance should be maintained between total resection and preservation of neurological function.
</dc:description>
