<dc:description>
Introduction: Chromatic aberration causes a depth-dependent mismatch between the visible diode aiming beam and the 1064-nm Nd:YAG treatment beam, which may be clinically relevant during posterior segment laser procedures. We report the clinical application of anterior defocusing of the aiming beam in the treatment of premacular sub-internal limiting membrane (sub-ILM) hemorrhage.

Case Presentation: A 23-year-old man developed acute bilateral visual loss in the setting of severe thrombocytopenia after failed bone marrow transplantation. Examination showed a large premacular sub-ILM hemorrhage in the left eye and a smaller premacular sub-ILM hemorrhage with associated intraretinal hemorrhages in the right eye. Presenting visual acuity was finger counting at 1 meter in the right eye and 0.12 in the left eye. The left eye underwent Nd:YAGmembranotomy using a Nidek YC-1800 laser with 300 µm of anterior defocusing and a single 2.0 mJ pulse. Immediate drainage of preretinal blood into the vitreous was achieved. At 10 months, visual acuity improved to 0.8 in the treated left eye, whereas the untreated right eye remained finger counting at 1 meter. Optical coherence tomography demonstrated a focal opening in the internal limiting membrane, with no structural abnormalities noted in the underlying retina.

Conclusion: This case illustrates the practical value of compensating for chromatic aberration during posterior segment Nd:YAG laser treatment. Anterior defocusing of the aiming beam may improve treatment precision and enhance procedural safety in selected cases of premacular sub-ILM hemorrhage.
</dc:description>
