          <dc:description>&lt;p&gt;Public healthcare providers routinely fail racialised and minoritised populations, producing not only systemic alienation, but also inequitable and unjust service operations outcomes. Framed as a management challenge with mapped interactions in Scotland, we identify a dominant vicious cycle in which insufficient professional cultural competence degrades the service ecosystem, eroding equitable access and creating systemic bottlenecks. Two further reinforcing loops deter service use: &lt;i&gt;Data Fear&lt;/i&gt;, driven by surveillance anxiety, and &lt;i&gt;Community Stigma&lt;/i&gt;, rooted in reputational harm. Both function as demand-suppression mechanisms, separating need from observed use in ways conventional capacity planning cannot detect. To disrupt these three interlocking cycles, we propose a phased implementation strategy anchored by a community-led &lt;i&gt;Software-as-a-Service&lt;/i&gt; reverse-training artefact embedding continuous professional development. Equity and justice should be treated as measurable dimensions of service quality, on a par with reliability and responsiveness, rather than peripheral ethical concerns.&lt;/p&gt;</dc:description>
