          <dc:description>&lt;p dir="ltr"&gt;Introduction: Thymoma is a unique tumour of mediastinal or ectopic thymus. They are more common in fifth and sixth decade of life and females are more commonly affected than males. Secondary degenerative changes such as haemorrhage, cyst formation, necrosis, fibrosis, calcification and ossification are known to occur.  Extensive fibro-osseous change is extremely uncommon.&lt;/p&gt;&lt;p dir="ltr"&gt; Case Presentation:  We report a case of 41-year-old male presented with difficulty in breathing and generalized weakness for two months.  Computed tomography (CT) scan revealed well defined lobulated soft tissue density lesion showing coarse calcifications in anterior mediastinum.  FDG PET CT revealed mediastinal neoplastic lesion. Histopathology of excised neoplasm revealed extensive fibro-osseous metaplasia and few loose clusters of epithelial cells with oval to elongate nuclei. IHC staining with p63, CK5/6 revealed squamous nature of cells. Considering clinical findings including radiological findings and histopathology features, a diagnosis of thymoma was rendered.  Tumour was staged according to TNM system approved by UICC/AJCC and Masaoka- Koga system. &lt;/p&gt;&lt;p dir="ltr"&gt;Conclusion: Thymoma should be considered in the differential diagnosis of anterior mediastinal lesions demonstrating extensive fibro-osseous or degenerative changes. In such cases, careful histopathology examination should be done to identify epithelial cells. Judicial use of IHC staining helps in diagnosis of thymoma.&lt;/p&gt;</dc:description>
