          <dc:description>&lt;p dir="ltr"&gt;Introduction: The retrorectal space is a complex anatomical region that can harbor diverse lesions, including rare congenital hamartomas like tailgut cysts and malignant tumors like chordomas. We report the highly unusual coexistence of these two entities in a single patient.&lt;/p&gt;&lt;p&gt;&lt;br&gt;&lt;/p&gt;&lt;p dir="ltr"&gt;Case Presentation: A 20-year-old woman presented with a 3-month history of lower abdominal pain. Imaging revealed a 45 mm retrorectal cystic lesion adjacent to the rectum. Resection was performed using a posterior Kraske approach with coccygectomy to improve exposure. Histopathological examination confirmed a tailgut cyst. Unexpectedly, examination of the resected coccyx revealed an incidental 3 mm microscopic chordoma within the medullary cavity.&lt;/p&gt;&lt;p&gt;&lt;br&gt;&lt;/p&gt;&lt;p dir="ltr"&gt;Conclusion: This case underscores the complexity of the presacral space and demonstrates that apparently uninvolved tissue removed for surgical access may harbor occult pathology. It emphasizes maintaining a high index of suspicion for malignancy even in young patients with benign-appearing cystic lesions.&lt;/p&gt;</dc:description>
