<dc:description>
Introduction : Currently, there is no standard first line chemotherapy for metastatic salivary ductal carcinoma (SDC), a rare, heterogeneous cancer with high mortality rates. Androgen receptors (AR) have been identified on majority of SDC patients, which makes it a potential therapeutic target using total androgen blockade (TAB), a treatment option already established for prostate cancer.

Case Presentation: This report explores a case of a patient who developed SDC post-radiotherapy for Hodgkin Lymphoma. He showed partial response to TAB before progression of one of the metastatic lesions. Molecular profiling by Guardant360 demonstrated that he had a mutation AR W742L which has been linked to therapeutic resistance in prostate cancer. The patient subsequently developed leptomeningeal disease which is extremely rare, especially secondary to SDC.

Conclusion: This study highlights a unique mutation AR W742L which has been associated with castration resistance in prostate cancer but little has been published about it in context of SDC. The case also reiterates the importance of being vigilant for secondary malignancies post-radiotherapy and adds to the growing evidence base for potential clinical benefit of androgen deprivation therapy for SDC.
</dc:description>
