Individual-level data cannot be deposited publicly because of the conditions of participants’ informed consent, institutional ethics requirements, patient confidentiality obligations, and applicable data-protection legislation, including the European Union General Data Protection Regulation (GDPR). Requests for controlled access to deidentified individual-level data are subject to the policies of the University Hospital Centre Sisters of Mercy, Zagreb, Croatia, and require the applicable institutional and ethics approvals.</p><p dir="ltr">The supplied plotting code is a reproducibility aid based on the final aggregate results.
