<dc:description><p dir="ltr">Fear of cancer recurrence (FCR) and fear of progression (FoP) are persistent concerns associated with distress, depression, anxiety, and reduced quality of life. Although related, FCR and FoP are distinct constructs, but previous reviews have rarely examined them separately or compared different types of psychological interventions. This systematic review and meta-analysis examined the effects of psychological interventions on FCR and FoP. We searched five databases and two grey literature sources for studies published between 1980 and August 2025. Two reviewers independently screened studies, assessed risk of bias using Cochrane RoB 2, and evaluated certainty of evidence using GRADE. Overall, 64 studies were included from 1,129 identified records. Psychological interventions reduced both FCR and FoP, with generally larger effects for FCR. Cognitive behavioural therapy–based interventions showed the most consistent evidence for reducing FCR.</p></dc:description>
