Despite comprehensive treatment with systemic antibiotics, intravitreal antibiotic injections, and pars plana vitrectomy, the patient's bilateral vision failed to recover. This case highlights the importance of enhancing surgeons' awareness of endogenous endophthalmitis as a rare but severe postoperative complication.
