<dc:description>
Introduction: Limited engagement of men in Primary Health Care (PHC) poses a challenge to comprehensive care and is influenced by sociocultural, organizational, and relational factors that affect access, welcoming care, and bonding. Objective: To identify and synthesize the actions and interventions used to improve men’s access to and welcoming care within health services, as well as the reported outcomes. Method: A descriptive and analytical systematic review conducted in accordance with JBI recommendations and reported according to PRISMA 2020. Searches were conducted in February 2025 and updated in March 2026 in PubMed, BVS, SciELO, Web of Science, Scopus, CINAHL, EMBASE, and Cochrane. Articles published in Portuguese, English, or Spanish were included, with no date restrictions, addressing men’s access, welcoming care, and/or bonding in PHC. Twenty-four studies were included. Results: Interventions addressing access included organizational flexibility in service delivery, active outreach, direct invitations, extended service hours, and the use of opportunistic contacts, such as health campaigns, prenatal care, reproductive planning, vaccination, and chronic condition follow-up. Regarding welcoming care, the main approaches included attentive listening, accessible communication, streamlined care pathways, less prescriptive language, and integration of clinical care, health education, and community-based actions. For bonding, educational groups, continuity of care, designated healthcare professionals, trust, shared responsibility, and interpersonal communication were identified. Conclusion: Promising evidence combined service reorganization, improved relational care, and recognition of men’s realities. The scarcity of evaluative studies reinforces the need for research on the effectiveness of interventions across different groups, contexts, and conditions, thereby strengthening comprehensive men’s health care in PHC.
</dc:description>
