<dc:description>
Insomnia is common among informal cancer caregivers but remains under-recognised and undertreated. This doctoral research aimed to characterise insomnia in this population, synthesise the evidence on accessible ways of delivering cognitive behavioural therapy for insomnia (CBT-I), develop a culturally and contextually tailored intervention, evaluate its feasibility and effectiveness, and examine how it was implemented and experienced among cancer caregivers in China.

Six studies were undertaken. A sequential explanatory mixed-methods study combined a cross-sectional survey of 427 cancer caregivers with semi-structured interviews with 20 of them, examining the prevalence of insomnia, associated factors specified in Spielman's 3P model, lived experiences and treatment preferences. Two systematic reviews with meta-analysis then synthesised randomised controlled trials of mobile health-delivered CBT-I (16 trials) and of self-help CBT-I (43 trials, 48 comparisons) in adults. These findings informed the development of SleepGuardian, a six-week WeChat mini program combining caregiver-tailored self-help CBT-I with weekly nurse support, which was refined through review by six domain experts and a single-arm mixed-methods feasibility study with 12 caregivers. Finally, a randomised controlled trial allocated 208 caregivers equally to nurse-supported SleepGuardian or sleep hygiene education, with outcomes assessed at baseline, six weeks and three months and analysed using linear mixed-effects models, alongside a mixed-methods process evaluation with 21 participants.
</dc:description>
