Each case integrates a &lt;b&gt;structured nine-field electronic medical record (EMR) narrative&lt;/b&gt; with a &lt;b&gt;paired panoramic radiograph&lt;/b&gt;, preserving the complete clinical reasoning chain from chief complaint through physical examination and imaging interpretation to diagnosis and treatment.&lt;/p&gt;&lt;p dir="ltr"&gt;Unlike existing dental AI datasets that provide image-only annotations or limited structured fields, this dataset captures the full diagnostic trajectory as documented by five practicing dentists in routine clinical care, reflecting authentic multi-physician practice variation.&lt;/p&gt;</dc:description>
