<dc:description>
Background: Obstetric complications are an important cause of severe maternal morbidity and mortality. Research describing the characteristics and outcomes of obstetric patients requiring intensive care in Nepal remains limited. This study aimed to describe the clinical profile, management, and outcomes of obstetric admissions to intensive care units (ICUs) in Nepal.

Methods: A retrospective observational study was conducted using data from the Nepal Intensive Care Research Foundation (NICRF) critical care registry. Women aged 15–49 years who were pregnant, postpartum, or admitted with obstetric-related diagnoses between 2019 and 2024 were included. Of 29,241 ICU admissions recorded in the registry, 257 obstetric patients met the inclusion criteria. Data was analyzed using descriptive statistics and univariate logistic regression.

Results: The mean age was 28.6 ± 5.8 years. Most patients underwent an operative procedure before ICU admission (76.3%), of whom 61.2% had an elective lower-segment caesarean section. Shock (19.1%) and severe pre-eclampsia (14.0%) were the leading reasons for ICU admission. Antibiotics were administered to 88.3% of patients. Mechanical ventilation was required in 30.0% of patients, vasoactive therapy in 17.9%, and renal replacement therapy in 2.3%. The median ICU stay was 2 days (IQR 1–3), and ICU mortality was 5.4%. Mechanical ventilation and vasoactive therapy were significantly associated with ICU mortality in the univariate analysis.

Conclusion: This registry study showed that obstetric ICU admissions in Nepal were predominantly postoperative and were mainly driven by shock and hypertensive disorders of pregnancy. ICU mortality remained low despite severe illness. These findings establish a national benchmark for obstetric critical care in Nepal.

</dc:description>
